On February 1, 2024, the Rotary Club of Owasso, in partnership with the Owasso Fire Department, proudly launched the Vial of Life program aimed at providing essential support for seniors, disabled veterans, and residents in need of emergency care.

This nationally recognized initiative equips emergency responders with crucial emergency medical information, enabling them to act swiftly, make informed decisions, and ultimately save lives when every second counts.

We’re on a mission to provide Vial of Life kits free of charge to residents in Owasso and neighboring communities. These life-saving kits can make all the difference in an emergency, and your support makes this initiative possible.

Note: A "housebound" disabled veteran is a veteran who is permanently housebound due to a service-connected disability or disabilities.

Vial of Life Kit

Each Vial of Life Kit contains:

Two Decals/Stickers,

One medical information form,

One zip-lock sandwich baggie from Sam's Club.

How it Works: (Steps displayed at the bottom of the page)

Residents should complete the medical information form by listing their health history and current medications. Do not include your social security number.

How to Prepare Your Vial of Life Kit:

1. Place your completed medical and insurance information form in the plastic bag.

Include copies of important medical documents, such as:

- Recent EKG (if you have a chronic heart condition)

- Advance Directives

- Driver’s license

- Veterans healthcare card

- Insurance cards

- Current prescriptions

- Medicare or Medicaid cards

- Signed Oklahoma Do-Not-Resuscitate (DNR) order

Seal the bag securely to keep everything protected and ready for emergency responders.

2. Place one sticker on the refrigerator door, taping the baggie to the refrigerator.

3. Place the second sticker on the front door at eye level.

Important Note:

If the resident needs to be transported to a hospital or urgent care facility, the Vial of Life kit should go with them. When emergency responders arrive, they can hand the kit directly to hospital medical and administrative staff. This ensures a smooth transfer of vital medical information, allowing caregivers to begin treatment faster and more effectively.

What we need:

Volunteers and donations are needed for this project. Donations can be made at the bottom of this website's home page or directly to the club treasurer. The free kits can cost an average of $.98 per kit (when three stickers are provided). Volunteers: Contact us at owassorotary@gmail.com.
