This nationally recognized program equips emergency responders with critical medical information, helping them act quickly, make informed decisions, and save lives when every second counts.

Each Vial of Life Kit contains:

Residents should complete the medical information form by listing their health history and current medications. Do not include your social security number.
How to Prepare Your Vial of Life Kit:
3. Place one sticker on the refrigerator door, taping the baggie to the refrigerator
4. Place the second sticker on the front door at eye level.
Important Note:
If the resident needs to be transported to a hospital or urgent care facility, the Vial of Life kit should go with them. When emergency responders arrive, they can hand the kit directly to hospital medical and administrative staff. This ensures a smooth transfer of vital medical information, allowing caregivers to begin treatment faster and more effectively.
