The Rotary Club of Owasso, in partnership with the Owasso Fire Department, launched the Vial of Life program on February 1, 2024, for home-bound seniors, housebound disabled veterans, and residents who may need emergency care. The Vial of Life program is nationally recognized as saving countless lives each year by providing emergency responders with life-saving medical information during an emergency.
We aim to provide the Vial of Life kits free of charge to Owasso and the surrounding community. The Owasso Rotary Foundation is a 501(c)(3) Nonprofit. All donations to the Vial of Life program are tax-deductible.
If your senior parent, housebound disabled veteran, or another family member had a medical emergency and were alone, they might be unconscious or confused. If so, who would provide the crucial medical information that EMS personnel need in an emergency? Even if awake and aware, people don't always remember details in a crisis. This is when the free Vial of Life kits can save a life!
Our goal is to help medical personnel make the best decisions regarding emergency treatment for Owasso, OK, and surrounding areas by providing a Vial of Life Kit for the residents who need it.

Note: A "housebound" disabled veteran is a veteran who is permanently housebound due to a service-connected disability or disabilities.

Vial of Life Kit
Each Vial of Life Kit contains:
How it Works: (Steps displayed at the bottom of the page)
1. The resident answers questions about their health history and medications on the medical information form. Do not add your social security number.
2. Place the completed medical and insurance information form in the baggie. Include copies of additional medical information such as: If you have a chronic heart condition, include a copy of your most recent electrocardiogram (EKG). Also include any relevant documents, such as Advance Directives, copies of driver's license, Veterans healthcare cards, insurance cards, prescriptions, Medicaid or medicare cards, and do-not-resuscitate order (signed Oklahoma document). *Securely zip the baggie closed.
3. Place one sticker on the refrigerator door, taping the baggie to the refrigerator, and one on the front door at eye level.
Important Note:
If the resident must be transitioned to the hospital or an urgent care facility, the Vial of Life kit travels with them. When emergency personnel arrive, the kit can be given to hospital medical and administrative personnel. This creates a seamless transition of medical information that allows hospital caregivers to provide needed services sooner.
